Terms and Conditions

1. Introduction Welcome to Polaris Rehab & Performance, a professional injury rehabilitation service. By using our services, you agree to abide by the following terms and conditions. These terms outline our policies regarding payment, session scheduling, cancellations, and other important information.

2. Service Offerings

  • Clients enroll in a monthly rehabilitation program, which includes either:

    • Two (2) sessions per month, or

    • One (1) session per month (available for long-term rehab clients based on availability and discretion).

  • Individual sessions may also be booked separately.

3. Payment Policy

  • Monthly clients will be charged automatically on the same date each month, based on the date of their first-ever session.

  • The billing month will begin on that date and continue on the same date each subsequent month.

  • Payments are non-refundable.

  • Individual sessions must be paid in full at the time of booking and are also non-refundable.

4. Appointment Rescheduling & Cancellation

  • Clients may rearrange their appointment with at least 48 hours' notice, subject to availability.

  • If there is no availability to rearrange, the session will be forfeited for that billing month.

  • If less than 48 hours' notice is given, the session will be forfeited for that billing month.

  • Missed sessions cannot be carried over to the next month.

  • No refunds will be provided for forfeited sessions.

5. Policy Enforcement

  • We reserve the right to enforce or waive any of these policies at our sole discretion.

6. Liability Disclaimer

  • While we provide professional rehabilitation services, we do not guarantee specific outcomes or results.

  • Clients are responsible for following provided guidance and attending scheduled sessions to maximize recovery.

  • We are not liable for any injuries, setbacks, or dissatisfaction resulting from participation in our programs.

7. Amendments to Terms

  • These terms and conditions may be updated at our discretion. Clients will be notified of any significant changes.

By enrolling in our services, you acknowledge and agree to these terms and conditions.
